In an effort to better serve you, below is a reminder check list of things to prepare before your first visit:

1.) Fill out an sign all downloadable
formsbefore your appointment.
2.) An original Arbitration Form and Informed Consent Form will be given to you to sign at the time of your appointment.
3.) Please allow approximately an hour and a half for your first visit and one hour for subsequent visits.
4.) Bring your new patient forms with you when you come in for your first visits: all new patient forms need to be complete before your first visit. Do not hesitate to discuss any questions you might have with us.
5.) Wear something very comfortable and loose fitting.
6.) Have something to eat prior to treatment (e.g. breakfast, lunch, or snack).
